Ferty9 Consent and Communication Notice

1. Consent modules

• Registration, enquiries and appointments.
• Portal terms and privacy acknowledgement.
• Clinical treatment, ART, cryopreservation, donor and procedure-specific decisions.
• Parent, guardian or authorised-representative authority.
• OTP, security and necessary service communications.
• Optional WhatsApp, SMS, email or marketing communications.
• Optional analytics cookies and similar technologies.
• Research or artificial-intelligence use only under a separately approved basis and notice.

2. What a consent record should evidence

• The person or verified representative and applicable role.
• Purpose, channel and specific choice.
• Notice or form version and language presented.
• Timestamp, source and capture method.
• Withdrawal or later change, where applicable.
• Verification evidence needed for the relevant risk, without recording an OTP or password.

3. Choice and withdrawal

Optional consent must be a genuine choice and should not be bundled with an unrelated purpose. You may withdraw an optional consent through the relevant preference control, communication instruction, centre or privacy contact. Withdrawal does not require deletion of records that Ferty9 must lawfully retain.

4. Clinical decisions

Clinical and ART consent must be obtained by the responsible care team using approved documentation after appropriate explanation. Applicable withdrawal rights, including before transfer of reproductive material where required, are handled by the clinical team and governing law.
